ADMISSION

 

Eligibility for Admission

  1. G.N.M. – Intermediate (Any Group with English) Minimum 45%
  2. A.N.M. – Intermediate (Any Group)
  3. Admissions strictly on merit basis.
  4. Age - 17 to 35 years on Dec. 31
  5. Medically fit for G.N.M. & A.N.M

Application Procedure

    1. Admissions are strictly on merit basis.
    2. Application forms are available at the student’s counter anytime during working hours on payment.
    3. The Application form is to be completed by applicant in her own handwriting.
    4. Admission forms complete in all respect together with following enclosures has to be submitted within 15 days after declaration of Intermediate result.

Documents required at the time of admission

  1. High school certificate with mark sheet
  2. Intermediate mark sheet
  3. Medical fitness certificate (including pregnancy test for married).
  4. Five colour passport size photographs.
  5. Character certificate.
  6. Transfer certificate

Documents required after the admission

  1. Caste certificate (for O.B.C. and SC/ST).
  2. U.P. domical certified.
  3. Anti-ragging certificate (From the side of student and parent)
  4. Income certificate (issued within six month)
  5. Marriage certificate for married candidate.
  6. Hepatitis -B vaccination certificate

Fee Details

  1. Fees are as per regulation of the U.P State Medical Faculty payable at the time of admissions.
  2. Admission is completed on the payment of the first installment of tuition fee along with other dues.
  3. A student joining the college after the commencement of the academic year will have to pay full fees of the first year in two installments, and for subsequent year payments on annual basis is realized.
  4. Fee must be deposited as per the fee schedule. Late payment fee of Rs. 500 per month will be charged.
  5. Fee once paid will not be refunded under any circumstances.
  6. Hostel fee is payable only by those students for full semester who will be provided hostel accommodation.
  7. Fee can be deposited in mode of cash or draft. (In favour of secretary KNMT sultanpur)

Institute Regulations

  1. Every student shall maintain high order of discipline have respect for teacher/institute and conduct in a dignified manner.
  2. Ragging is strictly prohibited.
  3. Students must reach the college 15 minutes before.
  4. Students are required to attend college in prescribed uniform and identity card.
  5. Student must treat the college property with great care and any damage and destruction caused to college property will lead to punishment as individual/penal as the case may be.
  6. Student must abide by the hostel rules and regulations of the institute.
  7. At the end of the academic session students will have to clear all their dues from all the department, library, labs, sport club and office. Admit card for the annual examination will be issued only after the clearance of all the dues.

Instructions for Hospital / Community Practice

  • Extreme care is required to keep any community member unhurt with her behavior/action.
  • The hospital rules are to be strictly avoided by all student.
  • The students are required to report 15 minutes before at the hospital during practice sessions.
  • All the equipments are to be carried out by the students for community/hospital practice.
  • It is the student’s responsibility to utmost care to keep away from any contamination. Similarly it is the responsibility to maintain your personal hygiene to keep any patient/community member unaffected.
  • The hospital rules are to be strictly abided by the students.
